There are some very clear references to Francis. In Chesterton's book, Francis literally "comes out of a cave walking on his hands". I obviously relate the song to what I'm personally going through as do most people so maybe it's just about addiction in general - the writer is probably the only one who truly knows. The Cave represents a dark soul.
